This session shows how prospective risk adjustment closes that gap, moving diagnosis and care gap insight directly into the provider's workflow from pre-visit chart review through point-of-care delivery inside the EHR.
You'll follow a clinical documentation integrity (CDI) nurse reviewing charts one to five business days before a visit. During the visit, the provider sees diagnoses for recapture, possible diagnoses for clinical consideration, and open care gaps. Accurate recapture at the point of care supports appropriate reimbursement, and each advisory links to the clinical evidence behind it, so a coder, compliance team, or auditor can trace every suggestion back to its source in the chart.
